Please be aware that hydrotherapy sessions cannot proceed without a signed referral form from your pet’s vet prior to their first appointment. This applies to all reasons for hydrotherapy. To submit your referral form to us, please download the relevant form below and email this to us by clicking here. Alternatively, you can bring the form with you before your pet's first session with us.
